The letter

[Date] To the GP [GP practice name] Patient: [Full name], date of birth [DD/MM/YYYY] Request for an ADHD assessment referral under NHS Right to Choose Dear Doctor, I would like to ask for a referral for an ADHD assessment. The main difficulties I have noticed: • [For example: I find it very hard to keep my attention on tasks and conversations] • [For example: I lose things and miss deadlines even with reminders] • [For example: I feel restless and act or speak without thinking] These difficulties go back to [childhood / school]. [For example: My school reports said I was bright but "needs to focus".] They cause problems with [work / studies / relationships / money / home life]. For example: [one specific consequence]. I can provide [school reports / an account from a parent or partner / work feedback]. If you agree that a referral is clinically appropriate, I would like to use my legal right to choose my provider under the NHS Choice Framework, and ask that the referral is made to [provider]. I understand they hold an NHS Standard Contract for this service with an Integrated Care Board in England, which makes them a qualifying provider. I have their referral form with me, or can send it to the practice. I am not currently receiving care from another NHS service for this condition. If you decide not to refer, please could you explain the reason in writing, so I can understand the next steps. Thank you for your help. Yours sincerely, [Your name]

Tips for making it work

If the GP says no

Ask for the reason in writing. A provider doesn't need a contract with your local ICB; a contract with any ICB in England qualifies. Read what to do next.
